Anomalies of the Paramesonephric duct is a group of congenital malformations that occur either from the failure to complete bilateral Mullerian duct elongation, fusion, duct canalization, or resorption of the septum. Septate uterus is the commonest of these anomalies and is associated with adverse pregnancy outcomes especially miscarriages. Metroplasty has been associated with improved pregnancy outcomes with hysteroscopic resection been the preferred approach currently. Open abdominal metroplasty is a useful alternative in low resource set-up or where facilities for hysteroscopy is not readily available or not affordable. The need for this case report is to document the case of a complete uterine septum in a 31-year-old woman with three previous miscarriages who had open abdominal metroplasty and subsequent pregnancy with good outcome.]]>

Analysis of the Factors Affecting the Male-Female Sex Ratio of Babies Born through Assisted Reproductive Technology.pdf /slideshow/analysis-of-the-factors-affecting-the-male-female-sex-ratio-of-babies-born-through-assisted-reproductive-technology-pdf/272250494 analysisofthefactorsaffectingthemale-femalesexratioofbabiesbornthroughassistedreproductivetechnology-241007222210-2ed195e0
BACKGROUND: Until now, the sex ratio in a population is believed to be relatively stable with no male/female preponderance. There has been an increasing amount of evidence to suggest that assisted conception may significantly impact on sex ratio (SR). Several factors have been suggested to affect SR such as parental variables (paternal race, maternal age, and body mass index-(BMI), methods of fertilisation (in-vitro fertilisation/intracytoplasmic sperm injection), stage of embryo transfer (cleavage/blastocyst), type of IVF cycle (fresh/cryopreserved), medications used for controlled ovarian stimulation, poor sperm motility, and even culture media. OBJECTIVES: This study aims to investigate the potential impact of the different ART procedures on sex ratio. It will also explore the relationship between paternal race, maternal age and body mass index BMI on sex ratio. METHODS: A retrospective cohort study from January 2017 to December 2023. Participants were women who had successful ART and delivery at Ninewells Assisted Conception Unit (NACU) Dundee and ART centre of Lagos University Teaching Hospital, Nigeria. RESULTS: Overall, 294 (66.2%) of the case records and 150 (33.8%) were evaluated from NACU and LUTH respectively. More male infants 244 (66.8%) were delivered following pregnancies conceived with blastocyst embryo stage transfer when compared with female infants which stood at 121 (33.2%). Concerning cleavage embryo stage transfer, 56 (70.9%) were in favour of female newborns while males accounted for 23 (29.1%). CONCLUSION: The study revealed that there is an increase in the proportion of male babies born following certain assisted conception techniques such as blastocyst stage embryo transfer and IVF while more female babies were born when cleavage stage embryos were transferred or when ICSI was used as a method of fertilisation.]]>

Modern Contraceptive Use at The Lagos University Teaching Hospital Family Planning Clinic /slideshow/modern-contraceptive-use-at-the-lagos-university-teaching-hospital-family-planning-clinic/272051253 jgrh-24-111-240927063324-3cb27dea
Background: Today, the importance of modern methods of contraception cannot be overemphasized. It even becomes more imperative in developing countries where maternal death during pregnancy is alarmingly high. Contraceptives, when properly used, will prevent most unplanned and or unwanted pregnancies subsequently lowering maternal illhealth and pregnancy-related deaths. This will invariably reduce the risk of unsafe abortion, HIV transmission from mother to neonates, improves education and wellbeing of the girl child and enhances women empowerment. To examine the pattern of modern contraceptive use amongst women attending the family planning clinic of Lagos University Teaching Hospital family clinic. Methods: This was a retrospective study carried out at the family planning clinic of the Lagos University Teaching Hospital between January 2021 and December 2022. Result: The mean age the women was 37.6(±5.7) years. They were mostly educated women (78.7% having a form of tertiary education). 51.7% have at least 3 children alive and 58.3% have completed their family size. Implants was the most accepted contraceptive (66.5%), followed by the intra-uterine contraceptive device (24.3%), injectable contraceptives (7%), oral contraceptive pills (2%) and the barriers (0.2%) in that order with female sterilization not being offered at the clinic where the study was carried out. Conclusion: In conclusion, contraceptive use was lowest amongst teenagers in this study. Subdermal implants being the commonest chosen method by the women followed by IUCD and injectables. ]]>

Epidemiology of Infertility and Characteristics of Infertile Women Requesting Assisted Reproduction Techniques in a Low Resource Setting in Western Nigeria /slideshow/epidemiology-of-infertility-and-characteristics-of-infertile-women-requesting-assisted-reproduction-techniques-in-a-low-resource-setting-in-western-nigeria/271868653 ugwu45102024ijtdh123288-240918073431-81f3ef64
Background: Infertility is an important gynaecological condition with psycho-social, cultural, economic, religious, demographic, and clinical consequences. Although the prevalence of couples having problems with conception has remained relatively stable worldwide, there has been an increase in the proportion of couples seeking fertility services. This substantial increase might reflect the improvement in the advancements in fertility care, improved success rates, increased availability of specialists and or involvement of insurance firms in infertility treatment. Paradoxically, countries in Africa with high infertility burden still lags in fertility care. This study was therefore aims to investigate the types and causes of infertility in patients attending fertility care in an assisted conception Centre in a public tertiary hospital in southwest Nigeria. Methods: This was a retrospective study of patient records who attended Assisted fertility centre (AFC) of Lagos University Teaching Hospital (LUTH), from January 1st, 2015, to December 31st, 2023. Categorical variables were analysed using percentages while continuous variables were analysed using mean and standard deviation and the association between categorical variables was analysed using chi-square test. The statistical significance level was set at p-value < 0.05. Results: The mean age and mean duration of infertility of the participants in the study was 40.89±6.8, and 8.14±3.5 respectively. About 72.9% had more than secondary education while almost two thirds of the participants had secondary infertility, 36.1% had previous pelvic surgeries, 11.4% had chronic pelvic pain and menstrual abnormality. Secondary infertility was more prevalent in women who has chronic pelvic pain, or those who has had previous pelvic surgery, previous infertility or previous abortion. Conclusion: In conclusion our analysis suggests that Secondary infertility is the commonest cause of inability to conceive in our facility. It also revealed that most of our women present late for fertility care at an advanced maternal age. ]]>

A Retrospective Audit of Placental Weight and Fetal Outcome at the Lagos University Teaching Hospital, Southwest Nigeria /slideshow/a-retrospective-audit-of-placental-weight-and-fetal-outcome-at-the-lagos-university-teaching-hospital-southwest-nigeria/271868587 aretrospectiveauditofplacentalweightand-240918073113-acc8e11b
Background: The placenta is an organ unique to mammals that connects the developing fetus to the uterine wall. The placenta supplies the fetus with oxygen and nutrients and allows fetal waste to be disposed of effectively via maternal circulation. Objective: The objective of this study is to determine the relationship between placenta weight at delivery and fetal outcomes. Materials and Methods: This was a retrospective study carried out between January 2022 and December 2023 at the labor ward of the Lagos University Teaching Hospital, Idi-Araba, Lagos, Nigeria. Results: The mean age of the women was 32.5 (3.4) years. The majority of the women were between 20 and 34 years of age. The mean placental weight was 657.5 (221.5) g. Maternal obesity and increasing parity were significantly associated with larger placenta weight (P = 0.04 and 0.01, respectively). Heavier placentas were also associated with fetal macrosomia and poor Appearance, Pulse, Grimace, Activity, and Respiration (APGAR) scores at 5 min (P < 0.05). Conclusion: This study concludes that a heavier placenta is associated with increasing parity, more operative delivery, bigger newborns, and a higher placenta-birth weight ratio.]]>

A Rare Case of Unilateral TrichorionicTriplet Tubal Ectopic Pregnancy Following In-Vitro Fertilization-A Case Report and Literature Review /slideshow/a-rare-case-of-unilateral-trichorionictriplet-tubal-ectopic-pregnancy-following-in-vitro-fertilization-a-case-report-and-literature-review-65df/271736603 tubalectopicpregnancy1-240911173206-0698f29a
comAbstractBackground:Trichorionictriplet gestation is an extremely rare type of ectopic pregnancy. However, with the increased availability of assisted reproductive technologies, the incidence of all forms of extrauterine pregnancy seems to be on the increase. Unlike singleton ectopic pregnancies where several management guidelines exist, little is known about high order/ multiple tubal ectopic gestations. Objective: To highlight case of a unilateral trichorionic triplet ectopic gestation conceived following invitro fertilization (IVF) and embryo transfer (ET).Patient: we present a case of a 40-year-old nullipara who had donor cycle IVF-ET which resulted in an extremely rare unilateral trichorionic triplet tubal ectopic gestation. She subsequently had a salpingectomy and a good outcome. Conclusion:This case report is a very rare condition of a unilateral trichorionic triplet tubal ectopic pregnancy after IVF treatment. Early ultrasound scan post embryo transfer in IVF-ET conceived pregnancies will help in early diagnosis and appropriate treatment. Reduction in the number of embryos transferred at IVF is a modifiable risk factor which could limit the possibility of this uncommon type of ectopic pregnancy. ]]>

BACkGRoUNd: Uterine fibroids, or Leiomyoma is a type of Smooth Muscle Tumors of the uterus (SMTs) and are common in the black race.Giant uterine fibroids, on the other hand, are uncommon and may occur during patient dissimulation. Dissimulation may occur because of adread of surgery and hospitals visits, fear of surgical death, chronic intake of herbal concussion, and a religio-traditional strong belief sys-tem on instant healing following prayers, among others. Myths like belief of defecating the uterine fibroids, some herbs that can melt themaway, and the belief that such illness may follow ancestral curses can fuel dissimulation. The surgical approach can be a source of chal-lenge, careful case selection considering the size and number of tumors can be helpful.CASe RepoRT: We present a 35-year-old nulligravida who presented to the clinic with a 14-year history of progressive abdominal swelling.Examination revealed a firm mass with a symphysio-fundal height of 55 cm. She subsequently had an open abdominal myomectomy with allthe myoma nodules weighing 12.9 kg in total! Histology confirmed uterine fibroid.CoNClUSioN: It is possible to offer open myomectomy in patients with giant uterine fibroid following careful patient selection with a con-sent for possible hysterectomy. Dissimulation can be minimized with repetitive counseling of patients. The choice of surgery depends on thesize and number of uterine fibroids, but surgical approach does not necessarily influence fecundity.keywoRdS: Dissimulation, fear of surgery, giant uterine fibroid, sub–Saharan Africa, surgical approach, Lagos]]>

The gains of invitro fertilization in alleviating the anguish of infertile couples cannot be quantified. However, despite these benefits, in-vitro fertilization increases the overall incidence of ectopic gestation. Twin tubal ectopic pregnancy occurs when two embryos or blastocyst implants in the fallopian tubes rather than within the endometrial cavity. Several mechanisms have been postulated to explain this rare occurrence. It could result from the splitting of embryos already in the Fallopian tube, or from dual implantation from multiple embryos transfer. The case report presented is that of a very rare unilateral twin tubal ectopic gestation following an in-vitro fertilization treatment which was diagnosed pre-operatively in a woman who has had a prior laparotomy with salpingectomy for ectopic pregnancy.]]>

Blighted Ovum, intrauterine viable gestation and Left Tubal ectopic pregnancy following Invitro-fertilisation- A case report and review of literature /slideshow/blighted-ovum-intrauterine-viable-gestation-and-left-tubal-ectopic-pregnancy-following-invitro-fertilisation-a-case-report-and-review-of-literature/270888772 bo-240809061114-7ffc97dd
Blighted ovum, otherwise referred to as anembryonic pregnancy, is a common complication following pregnancies conceived via invitro fertilization and embryo transfer (IVF-ET). It occurs when there is a gestational sac with a failed embryo development. On the other hand, the co-existent of viable intrauterine and ectopic pregnancy (heterotopic pregnancy) is rare. However, the advent of assisted reproductive technologies and treatment (ART) has increased the occurrence of heterotopic pregnancies. We present a case of heterotopic pregnancy alongside twin intra-uterine gestation one of which is viable and the other a blighted ovum with the presence of a co-existing uterine fibroid. She had laparotomy and salpingectomy for the ectopic gestation and conservative management of the intra-uterine pregnancies and an eventual live birth of a singleton baby at term. This case report highlights the importance of early ultrasound scan post ART in order to diagnose the different forms of pregnancy and manage them as appropriate]]>

Spontaneous Heterotopic Pregnancy After a Previous Ipsilateral Oophorectomy: A Case Report /slideshow/spontaneous-heterotopic-pregnancy-after-a-previous-ipsilateral-oophorectomy-a-case-report/270837281 nwogu712024ajrre121075-240807110649-1efc83c4
Heterotopic pregnancy occurs when there is coexisting intrauterine and ectopic pregnancies. The increased incidence of multiple pregnancy with ovulation induction and assisted reproductive technology (ART) increases the risk of both ectopic and heterotopic pregnancy. It has an incidence ranging from to 1:30,000 pregnancies of spontaneous conceptions to 1:100 to 1:500 with the use of assisted conception. It is a rare event with significant maternal morbidity and mortality due to misdiagnosis and late presentation especially following spontaneous conception where index of suspicion is very low. We present a very rare case of heterotopic pregnancy in a natural conception after a previous contralateral oophorectomy, who subsequently had salpingectomy with normal progression of the intra-uterine pregnancy till date]]>

Innovative Use of Padded-Condom Penile Mould Post-vaginoplasty in Low Transverse Vaginal Septum: A Case Report /slideshow/innovative-use-of-padded-condom-penile-mould-post-vaginoplasty-in-low-transverse-vaginal-septum-a-case-report/270623110 ugwu712024arjgo120258-240730154957-20814262
The case report below is that of an initially obstructive low transverse vaginal septum who had relieve of obstruction as an adolescent through an incision and drainage only to present with inability to achieve vaginal intercourse much later. Diagnostic dilemma was resolved by a paediatric Foley catheter passed through the septal dimple and the inflated bulb of catheter viewed as it is advanced along the vaginal canal. She had surgical septum resection by the double cross-plasty technique of vaginoplasty and vaginal dilator was fabricated as an adjunct to prevent stenosis. She had successful post operative period with no complication. She resumed uneventful sexually relationship that subsequently led to pregnancy after marriage. She is currently being followed up in our antenatal clinic. ]]>

Vaginoplasty for Acquired Gynaetresia Secondary to Insertion of Herbal Concoction in the Vagina: Successful Repair: A Case Report /slideshow/vaginoplasty-for-acquired-gynaetresia-secondary-to-insertion-of-herbal-concoction-in-the-vagina-successful-repair-a-case-report/270437430 adefemi712024ajpcb120269-240723134123-b04efc28
Acquired Gynaetresia is more common in developing countries mostly due to unconventional crude treatment involving per vagina insertion of various caustic substances for various gynaecological purposes leading to tissue injury, alongside possible superimposed infection from unhygienic local substances and eventual healing by fibrosis. This case reports brings to fore a case of 32-year-old woman with four months history of absent menses following insertion of corrosive in the vagina to treat premature ovarian failure. This caused her severe vaginal stenosis which was subsequently corrected by surgery and serial dilatation. ]]>

Vaginal pessaries are used in gynaecology to treat utero-vaginal prolapse or urinary incontinence (pelvic floor disorders). However, self-administered or provider-administered per vagina insertion of herbal materials and concoctions is a practice prevalent amongst the low socioeconomic class of developing nations with poor access to good comprehensive health care. These oftentimes unhygienic and toxic materials may present with symptoms of immediate and or long-term complications. Asymptomatic cases may be forgotten only to be discovered incidentally.
