2) Glucocorticoids reduce inflammation by repressing genes for inflammatory molecules. However, they can cause side effects due to systemic exposure.
3) Newer glucocorticoids are delivered directly to the lungs via inhalers to minimize systemic exposure and side effects while still reducing airway inflammation.

Role of ICS in Asthma and COPDGamal Agmy- ICS are the most effective controller medication for treating asthma and reduce symptoms, exacerbations, and improve lung function. Adding a LABA to ICS is more effective than increasing the ICS dose alone. ICS work by reducing airway inflammation.
- For COPD, ICS reduce exacerbation risk by about 18% but may increase risk of pneumonia. The effectiveness of ICS in COPD depends on eosinophilic inflammation - patients with elevated blood or sputum eosinophils experience greater benefits from ICS treatment.
