I create customized and/or general appeal letter templates to be used for denials of down-coding, medical necessity denials, bundling denials, and payments below standard fee schedule allowance. I also create full revenue cycle tool-kits specific to practice specialty that include coding & auditing for all service CPT's charged that includes payment and audit tools, general appeal letter templates, credentialing templates, collection letter templates, etc. For denial prevention and the reduction of denials, I perform Full Practice Medical Coding Profiles using the practice's encounter form that includes appropriate ICD-10 coding, modifiers, and CPT code combinations.
