This document discusses reinventing long term care in the Netherlands by increasing self-sufficiency for individuals with psychiatric and/or intellectual disabilities to reduce costs. It notes drivers of success include new intermediate services, a positive health approach, detailed measurements, and an attitude of just doing it. Challenges mentioned are that funding remains budget-driven, measurements need improvement, and there is no level playing field. It seeks a partner to turn incomplete measurements into an evidence-based package for long term residential health care in the Netherlands.
