Pemberian Cairan dan Perhitungan
Jumlah Cairan Elektrolit pada Luka
Bakar
Prinsip dari pemberian cairan pertama kali
adalah pemberian garam ekstraseluler dan
air yang hilang pada jaringan yang terbakar,
dan sel-sel tubuh. Pemberian cairan paling
popular adalah dengan Ringer laktat untuk
48 jam setelah terkena luka bakar. Output
urin yang adekuat adalah 0.5 sampai
1.5mL/kgBB/jam.
 Formula yang terkenal untuk
resusitasi cairan adalah
formula Parkland :
24 jam pertama.Cairan Ringer
laktat : 4ml/kgBB/%luka bakar
 Cara lain adalah cara Evans :
Luas luka bakar dalam % x berat badan
dalam kg = jumlah NaCl / 24 jam
Luas luka bakar dalam % x berat badan
dalam kg =jumah plasma / 24 jam
2000 cc Dextrose 5% / 24 jam (untuk
mengganti cairan yang hilang akibat
penguapan)
 Menggunakan rumus Baxter yaitu :
% x BB x 4 cc
 Menggunakan formula
Curreri, adalah 25
kcal/kgBB/hari ditambah
dengan 40 kcal/% luka
bakar/hari.
Pasien yang menerima resusitasi yang di
pandu menerima cairan lebih banyak secara
signifikan dalam 24 jam pertama setelah
terbakar dibandingkan dengan mereka yang
diresustasi menurut formula Parkland.
 Kejadian sistemik awal sesudah luka
bakar berupa: ketidakstabilan
hemodinamik ( perpindahan cairan,
natrium serta protein dari ruang
intravaskuler ke ruang interstitial akibat
hilangnya integritas kapiler) akibatnya
terjadi: penurunan curah jantung ,
selanjutnya mengakibatkan:
hipoperfusi dan hipofungsi organ.
