Training Leaders to Lead Highly Productive Teams

Are you responsible for leading a team (CEO, Board Chairperson, or Team Leader) or responsible for training staff (Training Manager or HR)? Research by Ruth Wageman indicates that "only 21% of teams are deemed highly effective, that leaves 79% rating as mediocre or poor." So then, why are 79% of teams underperforming? In my experience, team leaders face a multitude of problems, but these three are high on their list:
