Go to Google Docs


Log in


Click ‘Create new’


Select ‘Form’


Type the Form Title here


Type instructions here


Choose the Question Type


Choose Text first


We make this a required
question


Click ‘Done’


Here’s your first question


Hover over “Question 2” til it
gets highlighted


Click the “Edit” Icon


Choose “Text” one more time


Ask for their email address


Make it a required question


Then, click “Done”


Click “Add Item”


Choose the type of question
in this sub menu


Let’s choose “Multiple
Choice” at this time


Put your question here


Write Option 1 here


Then click the next button for
the next option


Write Option 2 here


In like manner, type the other
options you like


Click “Done”


There’s you menu


Click “Save” button


Now, what?


You can email the form


Type the email address


Click “Send”


This is the email your friend
receives


He clicks the link


This is what he will see


He fills it up


Then click “Submit”


He will get this confirmation


You can also embed
your survey in your
website


Click ‘More actions’


Click ‘Embed’


Press Ctrl-C (Windows)
or Cmd-C (Mac)


Go to your website


Paste the code
Ctrl-V (Windows) or Cmd-V (Mac)


Click Publish Page


There’s your survey form


Scroll down and see the
whole form


Your visitors fill this up


Then they click “Submit”


Then, how do you see
the responses?


Go to Google Documents


This file gets created
automatically. Click it.


You’ll find your responses in
this file, real time


Notice the Timestamp


Let’s rename the file. Click on
the existing name.


Give the file a new name


There’s your new file name


Cool form, isn’t it?
