Clinical treatment of children with ASD: empowering child and family. An Introduction Monique Verburg A step forward with autism
Basic questions to parents Reasons why child cant live at home now and is in need for clinical treatment Estimate if raising child at home could be possible in the future Inventarisation of conditions to make growing up at home possible in the future A step forward with autism
Reasons for clinical treatment Aggression Rigidity and being self-absorbed Need of intensive 24-hours support Apparent inability to learn Problems in all life domains A step forward with autism
Obstacles for living at home in future Parental fear of repetition of negative experiences Parents feeling incapable of answering their childs special needs Lack of appropriate school, mental health care or other necessary facilities Family problems Caregivers say raising child at home is not a possibility A step forward with autism
Necessary conditions for raising child at home able to control aggression more flexible and less self-absorbed self - management level more appropriate to calender-age parents feel confident and have more energy appropriate living environment, school, etc. BUT: assumption is often: not realistic A step forward with autism
Credo Our first and most important aim is helping families to feel capable to raise their child at home A step forward with autism
Basic Attitude Making a new start Tuning in on home-situation of child and family Focus: conditions needed to raise child at home Analysing developmental stagnation child-related-, family-related- and environmental factors A step forward with autism
Basic Attitude 2 Thinking out-of-the-box Breaking fixed patterns Supporting in developing new behaviour working on key problems helping child and family to develop their capabilities helping adapting the environment Cooperating intensively with the environment of the necessary conditions. = EMPOWERING child and family A step forward with autism
Basic Assumptions The environment (siblings, school, work, etc.) can be empowered to be able to adapt to the needs of the individual child A step forward with autism Child disruptive behaviour and handicaps and current inability of parents are changeable Children can learn how to integrate their vulnerabilities in their daily lives Parents can become adequately equipped to support the development of their vulnerable child
Treatment process Treatment focus = helping family to have their child back home Working together; partnership family-clinic Focus on home-setting Necessary changes = treatment goals A step forward with autism
Treatment goals child I am able to express my feelings in a proper way I am able to play on myself for half an hour I can be with my brother without excessive fighting I feel good about myself A step forward with autism
Treatment goals parents As parents we are in charge I am able to manage the aggression of my child properly I know how to stimulate my child doing something I want him to As (ex-) partners we are working together A step forward with autism
Environmental factors Appropriate school and leisure possibilities Support from family and friends Time-out setting for weekends and holidays Treatment can continue after leaving the clinic Quick access to appropriate help if needed A step forward with autism
Treatment aspects child Therapeutic living environment Psychoeducation Training modules Psychotherapy Medication A step forward with autism
Treatment aspects family Parents participating in patient-staff meetings Psycho-education Parental training modules Parental guidance by social worker Family therapy, FPA Parents participating incidentally in living group A step forward with autism
Potential of basic treatment method rigid patterns learned helplessness / l child parents environment Positive corrective experiences A step forward with autism
Potential of basic treatment method Can be used in various settings Practical, playful Motivation driven Evidence based Out of the box A step forward with autism
Results July 2008 May 2011 Outstream of 30 children: 15 went home (50%) (before 30%) 12 went to the adolescent clinical treatment settings of the LKH (40 %) 1 went back to the referring authority (3%) 2 went to a residential setting elsewhere (7%) A step forward with autism
Results until now Long term perspective of child and family is central issue Treatment focus is shared by all involved Parents feel confident with treatment vision Better cooperation of family and staff Waiting lists decreased and are in control More children are going home after treatment (but less than we hoped for) A step forward with autism
The essence is not in reaching our goal, but is the pathway we are walking together. A step forward with autism
