Community Level
Health extension
? Identifies and notifies
probable maternal and
perinatal deaths
? Reports from the
community to the
respective health center
surveillance focal
person within 24 hours.
? Completely fills verbal
autopsies within
2weeks after
notification
? HEW Summarizes a
total deaths and reports
to the respective health
center on a weekly
basis.
MPDSR ROLE AND RESPONSIBILITY
Health Facility level
Surveillance focal person at HC
? Immediately notifies (within 30
minutes) the PHEM focal person of
the respective Woreda
? Formally completes the
identification and notification
formats within 24 hours.
? Fills the verbal autopsy of all
deaths within one week of
notification.
? Receives weekly report filled &
reported by HEWs every week and
reports to the next level(Woreda).
? Completes FBMDA & FBPDA for
every death notified from the
facility within 1 week of initial
notification.
? The facility RRT will review
FBMDA & FBPDA and VA within
1week, and completes the
MDRF/PDRF and develops a
response action plan
? Maternal death reporting
form(MDRFs)/perinatal death
reporting form(PDRFs) will be
sent by the surveillance focal
person within 48 hours to the
immediate higher level PHEM
unit(woreda PHEM focal person).

Woreda Health Office level
PHEM officer at Woreda
? Works closely with the MNCH officer
for MPDSR/PHEM.
? Works with RRT/ERT(emergency
response team) led by the Woreda
administrator for multi-sectorial
response & management of
MPDSR/PHEM
? Receives weekly report forms filled &
reported from health centers on a
weekly basis and sends to the zonal
PHEM unit
? Receives MDRFs and PDRFs from all
health centers within one month
following receipt of weekly reporting
form (WRFs) and sends to the next
level.
? Checks the MDRFs and PDRFs
for completeness and sends to
the zonal PHEM unit.
? Compiles and analyzes WRF
and MDRF/PDRF data, and
produces a report.
? Works with the MNCH unit
and Woreda administrator to
organize a dissemination
meeting for multi-stakeholders
of the RRT/ERT/ to plan and
implement responses included
in the action plan.

PHEM officer (zonal)
? works closely with the
PHEM and MNCH officers
of the RHB/zone and
Woreda health offices for
MPDSR/PHEM.
? Receives WRF from
Woreda health offices and
hospitals on a weekly
basis & passes to Regional
PHEM unit
? Receives MDRFs/PDRFs
from all Woreda health
offices and hospitals
within one month of
receiving WRFs of
deaths.
? Checks for
completeness of the
MDRFs/PDRFs and
sends to the next level.

Regional Health Bureau level
RHB PHEM unit
? works closely with the MNCH unit of
the region for MDSR/PHEM.
? Leads the Regional MPDSR/PHEM TWG
in close collaboration with the MNCH
unit for response & management
? Works with the regional multi-sectorial
MPDSR/PHEM response.
? Receives weekly report form from
zonal & town health offices on a weekly
basis and reports to the National PHEM
unit.
? Receives MDRFs/PDRFs from all zonal &
town health offices within one month
of receipt of weekly report forms(WRFs)
of deaths.
? Checks MDRF/PDRF completeness and
sends the MDRF/PDRF copies to the
national PHEM unit, keeping one copy in
the regional PHEM unit.
? Compiles and analyzes weekly reports
filled in weekly report form( WRF) and
MDRF/PDRF data, and produces a report.
? Collaborates with the MNCH unit and
regional administrator to organize a
dissemination meeting for regional
PHEM multi-sectorial stakeholders to
plan and implement responses identified
in the action plans of the MPDSR TWG.
? Develops a report on monthly, quarterly,
bi-annual and annual plans of the
regional PHEM units

PHEM unit within EPHI
? works closely with the MNCH
unit of the FMOH of Ethiopia on
MPDSR/PHEM.
? Leads the MPDSR/PHEM TWG in
collaboration with the MNCH
unit for response & management
at the national level.
? Receives WRFs from regional
PHEM units on a weekly basis.
? Receives MDRFs from all regional
PHEM units within one month of
receiving WRF of the deaths.
? The national PHEM unit checks
MDRF completeness.
? Compiles and analyzes the WRF,
MDRF and PDRF data and
produces a report for the
development of an action plan by
MPDSR TWG.
? Collaborates with the MNCH unit
and FMOH higher officials to
organize a dissemination meeting
for national PHEM multi-sectorial
stakeholders to plan and
implement responses included in
MPDSR/TWG action plans.
? Develops report on a monthly,
quarterly, bi-annual and annual
basis
