This document is a nomination form for the 2010 Eli Segal Award, which recognizes outstanding entrepreneurial spirit and innovation in national service. The form provides instructions for nominees to submit their contact information, a description of their project or organization, and narrative responses that address how their work demonstrates vision, community building, connecting people, active service, humility, community impact, use of technology, and sustainability. Nominees are also instructed to submit at least one letter of endorsement by the stated deadline.

AmeriCorps Alums 2010 Eli Segal Award Nomination Form All fields must be completed in order to evaluate your nomination. Please fill out the entire nomination form below. NOMINATION FORM: PLEASE BE SURE TO ANSWER ALL QUESTIONS THOUGHTFULLY AND THOROUGHLY. FEEL FREE TO USE MORE SPACE THAN IS PROVIDED. Nominees Project, Organization or Business Description Please give us a description between 50 and 100 words that outlines the nominees entrepreneurial endeavor (project, organization, business or idea). Entrepreneurial Spirit How has the nominee demonstrated outstanding vision with regard to innovative approaches in building community through service? How did the nominee realize his/her vision despite barriers or difficult odds?
How might the innovation developed by the nominee help advance the field of national service? Specifically, how has it contributed to the growth and advancement of the AmeriCorps Alums movement? Convener & Connector How does the nominee bring people together to reach a common goal? Please address how this person connects to other AmeriCorps members and alums in his/her community. Action How is the nominee active as an organizer, advocate, and/or agent of change in their community? In their city, state, US or world?
Service How does the nominee promote and participate in service and volunteerism within and beyond his/her community? How has the nominee advanced the call to service? What is most impressive about the nominees commitment to service? Humility How has the nominee shown that his/her efforts are focused on benefiting the greater good and not serving individual interest?
Success and Impact How has the nominee empowered or improved the conditions of the community and its citizens? What results have been achieved thanks to the work of this nominee? Explain how the nominee has implemented innovative use of information and technology. Sustainability How has the work or initiative of the nominee rooted itself in the community? Explain how the initiative will/would sustain itself in the absence of the nominee.
Letters of Endorsement Letters of endorsement play an important role in determining the strength of a nomination. While only one letter is required, multiple letters of endorsement will be accepted. All letters of endorsement must include the nominees name and contact information and should be submitted by the endorser to EliSegalAward@americorpsalums.org. Letters of endorsement should address the aforementioned judging criteria. Letters of endorsement must be RECEIVED by 5pm EST on Tuesday, August 31, 2010 via email to be considered. Email to: elisegalaward@americorpsalums.org with the candidates name in the subject line like this: Subject: Letter of Endorsement for (insert your nominees full name) Marketing/Promotional Samples Brochures, flyers, or other promotional material may be sent electronically with the Nomination form via email.
