- 7. Enter your EHR Incentive Program Registration Number (given to you when you completed Registration)
- 8. Enter the 15-digit ONC Certification Number For SOAPWARE 2011 (30000001TQQIEO)
- 9. Type in a narrative Justification
- 10. When completed, Attest and Submit
