Can I get my routine annual physical and my biometrics screening for the
PHA done at the same time? If yes, what else do I need to do?
Yes, you can have your physician complete your routine annual physical and your
biometric screening at the same time. However, in order for both components to
count, they must be completed between April 1st, 2012 and June 30th, 2013. In
addition, if you are opting to have your biometric screening done by your physician
rather than attending an onsite biometric screening you must have your physician
complete the PHA Health Screening Verification Form.

Can my Spouse/LDA take the PHA?
Spouses currently on the Bon Secours Medical Plan are welcome to participate in the PHA.
They are welcome to attend any of the onsite biometric screenings and can log on to
bonsecourswellness.com to complete the online wellness profile. The Spouse/LDA ID is the
employees Lawson # plus S (e.g. 00000S). The initial password is the Spouse/LDA date of
birth (format mmddyyyy, no slashes or spaces).

If your Spouse/LDA completes both parts of the PHA (biometric screening and
online wellness profile) you will earn a $300 pre-taxed cash award on the Bon
Secours employee pay check
What if Im on a Leave of Absence during this years PHA?
If you are on a Leave of Absence during the PHA period, you can still go online to
complete your online wellness profile and you can have your physician complete
your biometric screening. Your physician will have to complete the PHA Health
Screening Verification Form.

Can I attend an onsite screening at another local system?
Yes. You can check the biometric screening schedule for other local systems on
the bonsecourswellness.com home page by clicking the Calendar link at the top of
the page.
Do the biometrics that my PCP took during my routine annual physical
count toward meeting the biometric screening requirement for the PHA?
Yes, as long as the biometrics were performed between April 1st, 2012 and July
31st, 2013 and you complete and submit a PHA Health Screening Verification form

Employees may get the biometric screening at the Good Help Clinic. Spouses
must attend one of the in-person screenings in order to be eligible for the $300
cash award.

I took the online wellness profile last year. Do I have to take it again this
year?
Yes, you need to complete the online wellness profile each year that you want to
earn the PHA wellness incentive. If your spouse is on the Bon Secours medical
plan and participating in the PHA this year, make sure they do the on-line profile
before the in-person screening. To find more information about spouses
completing the on-line profile refer to the BSV How To Wellness Profile 2013 on
the Motivation Alliance Blog.

screening on-site and not with PCP

WHY: Know your Numbers! Spouse/LDA on the Medical Plan who complete
both parts of the PHA, will earn a $300 Cash Incentive (pre-tax and issued in
BSHSI paycheck after September 2013)

How long will it take for my routine physical to show up on
bonsecourswellness.com?
If you use a in-network provider that files the claim on your behalf, it could take 2
months or perhaps longer depending on when the claim is incurred versus when it
is submitted and processed. The longest delays occur when the physical is
completed at the very end of a month but isn't submitted and received by the
medical carrier until the next month. Prior to the badge showing on
bonsecourswellness.com, if you are a UnitedHealthcare member you can login to
myUHC.com and verify that the claim is showing in your history and that it was
paid at 100% with no copay (indicating that it was processed as a routine annual
physical).

How do I know if my physician coded my claim correctly?
Check to see if you've earned the Routinely Healthy badge on
bonsecourswellness.com. Click on Badges Earned under your Profile or click on
Our Incentive Program under the Discovery section.
If your badge isnt showing up and you are a UnitedHealthcare member you can
also login to myUHC.com and verify that the claim is showing in your history and
that it was paid at 100% with no copay (indicating that it was processed as a
routine annual physical and the claim just hasnt been loaded into
bonsecourswellness.com yet).
Does my physician have to fill out a form when I get my physical?
The only time a form will need to be completed is if your physician doesnt code
your visit as a routine annual physical or in some cases when you use an out of
network physician. Go to IRIS, Human Resources, Benefits and click on the

What if Im pregnant and will have a BMI over 27.5 for this years PHA?
The best course is for you to complete 50 Daily Challenges to earn your Healthy
Weight badge. If you are physically unable to complete any of the options to earn
the Healthy Weight badge you can file an appeal after July 31st.

Can I skip the online wellness profile so that I can access the other features
of bonsecourswellness.com (e.g., the Daily Challenge)?
You can't skip the online wellness profile if you log in using your computer.
However, if you have a smart phone and you download the
bonsecourswellness.com app (see instructions below) you can get to your home
page without completing the online wellness profile.

and be prepared to take your shoes off.
You can attend screenings at any location and
we will try to get you in and out as quickly as
possible. Please plan a few extra minutes
because your biometrics will be uploaded into
the portal immediately.
While waiting you will be asked to complete a
communication survey that evaluates the current
avenues being used by Employee Wellness.
Once you complete the screening and survey,
you will be entered into a prize drawing.
If you cannot make it to one of the screenings,
you can complete the biometric screening with
your physician. Be sure to print off the form to
take with you which is located on IRIS, under
benefits or on the Wellness Website under forms

21. How can leaders help?
