A 55-year-old woman presented with symptomatic varicose veins in the lower extremities and postural alteration associated with laparocele. Doppler ultrasound examination showed a patent and phasic deep venous system, patent and continent superficial veins, and incompetent valves in the right greater saphenous vein with reflux distally. On the left, there were signs of previous stripping of the greater saphenous vein in the middle third of the leg with crural recurrence due to neovascularization. Surgery was planned for the right greater saphenous vein.

A 48-year-old man with a history of bilateral varicose vein sclerotherapy presented with symptomatic left lower extremity macrovarices and stasis dermatitis at risk of ulceration due to previous local trauma from sports. In January 2013, he underwent conservative endovenous ablation of the right great saphenous vein using CHIVA with minimal phlebectomy of the thigh and mid-calf junctions. Doppler ultrasound showed a patent and valvular deep venous system, competent external saphenous veins, and reflux from the left saphenous valve terminal and left femoral vein.
