This document is a standing instructions form to authorize Bajaj Allianz Life Insurance Company Limited to debit renewal premiums from a credit card as they become due for certain insurance policies. It lists the credit card number and expiration date, policy numbers, premium amounts and due dates. The card holder provides their signature to authorize the automatic debits. Terms and conditions specify that the credit card must be in the policy holder's name, photocopies of both sides of the card are required, and the authorization will remain in effect until notified otherwise.

Bajaj Allianz Life Insurance Company Limited Head Office: G.E.Plaza,Airport Road, Yerawada, Pune-411006 STANDING INSTRUCTIONS FORM - CREDIT CARD Please fill and submit this to the nearest customer care centre. I hereby authorize Bajaj Allianz Life Insurance Co Ltd., Pune to debit my credit card number for the renewal premiums due on my policies as and when due and which is mentioned in this standing instruction form (hereinafter referred to as SI form) Credit Card No.- Expiry Date (mm/yy): Card Type VISA Master Date of birth (As per credit card) Issued By (Name of Issuing Bank) The policy numbers for which this standing instructions are given are as follows: - Policy No Name of the insured person Tel. Mobile Email Relation of insured person with card holder Amount of Premium Rs. Monthly Quaterly Half yearly Annual Due dates of renewal premium I understand that the above transfers will be executed as per the terms and conditions below. Card Holders Name ( As appearing on credit card) Card Holders Signature Policy Holders Signature (As appearing on credit card) (If Diff from credit Card Holder) Terms And Conditions 1. This form may be used in conjunction with regular premium policies and supercedes any previous instructions held 2. The Credit Card should be in the name of policy-holder . 3. The third party payment will not be accepted. 4. Photocopy of the both sides of Credit Cards should be furnished along-with this Direct Debit Authorization. 5. Direct Debit to the credit card would take place as and when the renewal premium will get due. 6. In case the transaction being declined, the policy holder is liable to pay the outstanding premium by cash or cheque failing which the policy will become lapse due to non payment of premium in due time as per the terms and conditions of the policy issued by Bajaj Allianz Life Insurance Co Ltd. 7. Authorization will remain in effect till intimated otherwise by the policy-holder or declinature of a mandate. 8. If there is some change or renewal of the Credit Card it should be intimated one month before to Bajaj Allianz Life Insurance Co Ltd. 9. This is an additional facility for payment, however as per the policy conditions the premiums are required to be paid on the due date. 10. If payment date falls on Sunday or Public holiday the same will be effected next working day. 11. In case of Free look Cancellation the amount will be refunded in the credit card account. 12. When your current card expires or replaced, please notify us by completing a fresh standing instruction form. Every Month
