The fourth ventricle is a broad tent-shaped midline cavity located between the cerebellum and brainstem. It develops from the cavity in the rhombencephalon portion of the neural tube during embryology. The fourth ventricle has lateral and medial recesses that extend into the cerebellum. It is bounded superiorly and inferiorly by the superior and inferior cerebellar peduncles, and laterally by the gracile and cuneate tubercles. Common lesions of the fourth ventricle include medulloblastomas, ependymomas, and astrocytomas.

#5: The AICA was responsible for supplying the plexus near the cp angles, as well as a portion of the lateral recess of the fourth ventricle into the lateral apertures.
The PICA supplied the choroid plexus in the median aperture and roof of the fourth ventricle.

The lateral recess is a unique structure communicating between the ventricle and cistern, which is exposed when treating lesions involving the fourth ventricle and the brainstem with surgical approaches such as the transcerebellomedullary fissure approach
In addition of a lateral recess incision to cerebellomedullary fissure dissection facilitates cerebellar hemisphere retraction.
