HeartToHeart is a healthcare collaborative that allows patients and providers to securely share data, communicate, coordinate care, and collaborate through a network. It offers remote patient monitoring, virtual consultations, and integrates clinical device data to provide coordinated care management with the goals of improving access, reducing costs, and improving outcomes. HeartToHeart is looking to partner with other organizations to pilot its remote monitoring and virtual clinic solutions to demonstrate lower total healthcare costs through preventive care and management of chronic conditions.
