- 8. Copy of current paid tax receipts (parish and city);
- 9. Copy of valid state ID
- 10. Copy of social security card if using personal name or; Copy of Employer Identification Number (E.I.N.) as required by Internal Revenue Service (I.R.S.) if using business name
- 12. Management agreement if owner does not manage his/her own property
- 13.
