This document provides guidelines for managing ST-segment elevation myocardial infarction (STEMI). It recommends using nitrates cautiously for hypotension, beta blockers like metoprolol for most but not for heart failure or bradycardia. Dual antiplatelet therapy with aspirin and ticagrelor is advised along with anticoagulants like heparin or enoxaparin. High potency statins and coronary reperfusion through PCI within 90 minutes or fibrinolytics if PCI unavailable are also recommended.

Nitrates • Caution with hypotension (eg, RV infarction)
Beta blockers
• Oral cardioselective (eg, metoprolol, atenolol)
• Contraindicated in heart failure & bradycardia
Antiplatelet
therapy
• Aspirin & P2Y12 receptor blocker (eg, ticagrelor)
Anticoagulation • Unfractionated heparin, enoxaparin, or bivalirudin
Statin therapy • High potency (eg, atorvastatin, rosuvastatin)
Coronary
reperfusion
• PCI ≤ 90 min from first medical contact
• Fibrinolytics (eg, alteplase) if PCI is unavailable
