This document is a monitoring tool used to track the performance of students and teaching methods used by an intermediate/secondary school teacher over a month. It contains tables to record the number of students who achieved mastery of class material, the methods and activities utilized by the teacher, and spaces for signatures from the teacher, department head, and principal. The tool is used to evaluate student learning outcomes and the teaching approaches employed on a monthly basis.
