This document outlines an individual's personal wellness plan. Their current diet consists of eggs, salads, meat and vegetables for meals, and fruits and jerky for snacks. Their goals are to exercise 3 days a week, remove processed foods, and incorporate more vegetables. They intend to accomplish these through goal setting, stimulus control, and chain breaking.
