STEP-1
ï‚— Make an Affidavit
ï‚— This can be done on Rs.10/- or higher value stamp paper
by the applicant in English
ï‚— Get Notarised
ï‚— If you are changing surname which is not being used by
your father or mother, the affidavit has to be signed by
either Tehsildaar or MRO or SDM or Executive
Magistrate
ï‚— Affidavit has to be signed by appicant
ï‚— In case of name change of Minor father or mother will
sign.
STEP-2
ï‚— Advertise name change advertisement in newspaper
ï‚— Only one newspaper is required
ï‚— The newspaper should be National Newspaper
ï‚— Although one copy is required, buy 5 or more copies
for future reference.
STEP-3
ï‚— Prepare formst as per requirement i.e. name change,
public notice, adoption, gender change or religion
change
ï‚— Attach self-attested Id and address proof
ï‚— Government employees should also submit Poll Deed
in the prescribed formt
ï‚— CD as per the format prescribed by Deptt. Of
Publication, New Delhi.
ï‚— One copy newspaper
STEP-4
ï‚— Pay Government fee for publication of advertisement
in the Gazette of India.
ï‚— Payment should be done online (bharatkosh.gov.in)
ï‚— For publication charges visit department website for
latest rate of advertisement charges
ï‚— All details about payment and fee available on
deptpub.nic.in
ï‚— Attach fee receipt along with your file
STEP-5
ï‚— Submittion of application may be done either by hand
or by post.
ï‚— Always prefer Speed Post than Courier
ï‚— Track you application on postal department website to
know whether your application has reached.
ï‚— In case of personal submission visit office on any
working day (Mon-Fri). Keep in mind government
holidays.
ï‚— Office timings are 10 am to 5 pm to submit application.
STEP-6
ï‚— Every Saturday Gazette is uploaded in PDF format
ï‚— Visit egazette.nic.in
ï‚— Download the latest gazette
ï‚— Check whether your name published.
ï‚— If published, print first page, last page and the page
where your advertisement appears.
