This training document outlines procedures for packing and documenting medications when clients take leave from the facility. Staff should pack each medication dose in a separate ziplock bag labeled with the dosage time and complete a visitor pass form. On the MAR, staff should initial and circle their initials for each scheduled dose while the client is away and note "resident is on leave" on the back of the MAR each shift. Following these procedures ensures medications are properly packed and the MAR accurately documents doses while clients are on leave from the facility.
2. Objective
Introduction-After the completion of the training
entitled Packing Medications staff should be able to
demonstrate the procedures and document the forms
correctly without error.

3. Packing Medications
All Medications not packed:
All the clients medications was not sent home during the
holiday vacation.
During the weekend home visits clients medications were
not sent home.
Solutions:
Staff will pack the medications in separate zip lock bags
with a written note per dosage times, i.e. 8am, 2pm, 5pm
and or 8pm AND complete a Visitor Pass form.
8. Documenting MAR
The staff that performs the duties for the shift while
the client is on leave will documented by completing
the following:
Only initial and circle your initials
Note: only initial and circle the date the medication
should be given during the time slot the medication
should be given.
9. Documenting MAR
Document on the back of MAR:
the reason(resident is on leave) this should be
printed each day/time the client is on leave.
Note: this should be completed each shift.
