- 2. I.INTRODUCTION The joke is that you are not a Filipino if you do not have a TB. It might soundamusing but it gives you a glimpse of how prevalent this disease is. In the late 1990’s,the Philippines was fourth in the world for the number of cases of tuberculosis, and hadthe highest number of cases per head in South East Asia. Today, there has been someimprovement but a lot still need to be done. Among the 22 countries in the worldaccounting for 80 percent of TB worldwide, the Philippine is now ranked no. 9. Almost 75of Filipinos die everyday because of TB. Almost everyone gets vaccinated with BCG asa child, and yet, this does not ensure that you will develop TB later. Is the DirectObserve Treatment Strategy (DOTS) working? To a certain extent yes, and only if thosewith symptoms consult immediately. The problem is that most Filipinos ignore their symptoms, continue to roam around and spread the infection, and consult only whenthere is blood coming out when they cough. Also, over the years, no one has developeda better vaccine and a better class of drugs against this infection. Meanwhile, the multidrug resistance capability of the organism due to mutation continues to progress.Primary Koch’s infection or primary tuberculosis is defined as infection of anindividual lacking previous contact with or immune responsiveness to tubercle bacilli. Inprimary lung infection, a single lesion (known as Ghon’s focus) is usually foundimmediately subjacent to the pleura in the lower part of the upper lobes or upper part of the lower lobes of one lung, rarely elsewhere. These localizations reflect the areasreceiving the greatest volume flow of inspired air.
