The subordinate is asked to rate their overall performance fulfillment of roles and responsibilities, how their performance aligns with their supervisor's expectations, how effectively they get their work done, whether their supervisor would change how they perform their job, and their overall competence as an employee. The form cites a 1997 journal article on perceived organizational support and leader-member exchange relationships.
