This document contains a 20 question survey for employees about their work environment and job satisfaction. The survey asks employees to rate their level of agreement on a scale of 1 to 4 with statements about their job appreciation, resources, stress levels, understanding of company objectives, teamwork, clear roles and responsibilities, feedback and evaluation from supervisors, care and recognition from colleagues, opportunities for growth, and an open work environment. There is also a section at the beginning to provide general information about tenure and department for analysis purposes only. The document concludes by thanking participants for their time.
