This document discusses informed consent and refusal of treatment issues. It begins by defining informed consent as a process where a health care provider discloses appropriate information to a competent patient so they can voluntarily accept or refuse treatment. It notes that consent was not historically required, as professionals were expected to determine treatment themselves, but it became important in the 20th century. The document then outlines the legal requirements for informed consent, including adequately informing patients and obtaining consent except in emergencies. It discusses standards for informed consent, including the professional standard of disclosing what colleagues would and the reasonable person standard of disclosing what patients need to make an informed choice. The document analyzes cases involving these standards and issues like ensuring understanding and voluntary consent.
To meet client expectations, analyze what they want by putting yourself in their shoes, charge a fair price for your work, and give your best work on time while being available to listen to any concerns.

Create the design for Persib Bandung
STORY
Persib the champions of the Indonesia Super League 2013/2014.
Persib, an acronym for Persatuan Sepak Bola Indonesia Bandung is an Indo-
nesian football club based in Bandung, West Java. In most media, the tau-
tology Persib Bandung is often used. Persib is the result of a merger of 3 foot-
ball clubs in 1933, namely Bandoeng Inlandsche Voetbal Bond (BIVB), Per-
satuan Sepak bola Indonesia Bandung (PSIB) and National Voetball Bond
(NVB).
Referring to the number of title in a long history of Indonesian football com-
petition, Persib is considered as one of the most successful clubs in Indone-
sia.
The 13/14 season of the Indonesian Super League, I'm responsible for the
design of the Persib Football Club team kit. This club represent the West
Javanese people and the city of Bandung in perticular (my second home).
So it's a great honor for me to design the team kit jersey of Persib Bandung.
BRIEF
Create the design for Persija Jakarta
STORY
Persija Jakarta is an Indonesian football club from Jakarta. It is one of Indo-
nesia's most successful clubs and the most popular sports club in the city.
Persija Jakarta was founded on November 28, 1928, with the embryo called
Voetbalbond Indonesische Jacatra (VIJ). VIJ is one who co-founded the
club Football Association All of Indonesia (PSSI) with the participation of rep-
resentatives VIJ, Mr. Soekardi in the formation of Societeit PSSI Hadiprojo
Yogyakarta, Saturday, April 19, 1930.
It is a Great honor to design their team kit for the 13/14 Indonesia Super
League. They have an unsuccesful 12/13 season. The bold and a more de-
tailed design is to give them more support to gain greatness by becoming
the champion of the Indonesia Super League.

Persib met their glory in the 80's, they even had a friendly match against
big clubs like AC Milan and PSV Eindhoven. The 14/15 season Home kit is
inspired by the 80's Jersey when they had their glory times. A simple all
blue kit with a simplified "maung" (tiger) pattern forming a "V" on the
body. I also added their nickname "Pangeran BIru" (Blue Prince) detail on
the inside of the collar.
The red line on the body, and n the arm cuff area represent the color of
our nation's Flag. It represent the Indonesian Flag, in the hope that the
player and the club could represent our nation in the international stage
and a proof that Persib Bandung is one of our Nation's pride.
The away kit is a traditional all-white jersey with blue trim on the collar
area and the arm cuff. Still with a watermark simplified tiger pattern on
the chest.
As mentioned before, the simplified "Loreng Maung" (tiger pattern) on
the body formed a "V" shape that represents the word "Victory". On the
side of the body there's a graphic element that inpired by the jersey that
they used on their last season when they win the league.
On the fight for our independence, our country fought tirelessly. This fight
for independence simultaneously happened through out the region in
Indonesia, including Bandung. The most iconic event that happened in
Bandung (which is the home of Persib) was The "Bandung Lautan Api" this
is the event when the people of Bandung burn their home, gave up all
their belongings to burn with the city, so that when the city is occupied by
the colonials, they will nothing but ashes.
The spirit of the "Bandung Lautan Api" represented by the camouflage
pattern on the third kit, the red detail on the colar area, represent the fire,
the spirit of the Bandung people.
The vanishing red graphic on the side of the shirt, represents the fire
comes from within, and the fire of "Bandung Lautan Api" Event.
The home kit is inspired by the 2001 kit where they became the champi-
on of the Indonesian Super League. On the right bottom of the body
there's a graphic with a list of championship title year with a circle
around it that inspired by the "Bundaran HI". We use a reflective material
on that graphic.
There's a a detail that represent Indonesia in the inside of the Collar area,
the red and white represents our nations flag. Persija is one of the most
decorated club in the history of Indonesian Football. The club itself is rep-
resentation of our nation pride.
The tiger print on the colar and in the inside of the arm cuff area is a rep-
resentation of the clubs nickname "Macan Kemayoran" (Kemayoran
Tiger).
The Away kit is simple jersey inspired by the early Persija kit when they use
an all-white away jersey.
The tiger watermark on the body is inspired by the club's nickname
"Macan Kemayoran" (Kemayoran Tiger) The line on the side of the body
inspired by the scratch made by a tiger.
There's also an added detail on the badge area, embossed application
adds a bolder look.
The early years of Persija they use red as their main color, not until in the
late 80's that they changed their main color into orange. The simple
design with pinstripe, added year of champion detail on the pinstripe.

BRIEF
Labbola is an independent company that handles football sta-

in japan and europe to promote their vision and idea in football
statistics. They requested me to design their logo and corporate
branding
STORY
The logo concept is inspired by football, and the pie chart, the
pie chart represents the data statistics that the company is
focusing. So I combine the two and made the design based on
that idea.
