This document contains a rating sheet for observing teachers at Aliwagwag Elementary School in Cateel 2 District. The rating sheet contains 9 indicators for evaluating teachers on a scale from 3 to 7. The observer is instructed to rate each indicator individually based on how well the teacher performed during the classroom observation and to attach their observation notes to the completed rating sheet. Both the observer and observed teacher must sign the completed form.
