One option is find a employer that has presence in your area and then get a US transfer.
Deloitte, Accenture, PWC etc have Healthcare verticals and are good employers. At this moment, I don't have any more information to help you. Spend more time in system section as 40% questions comes from this section. I have shared cphims practice QA , don't try to memorize it, see all option and try to understand reason behind each right and wrong option.

Spend more time in system section as 40% questions comes from this section. I have shared cphims practice QA , don't try to memorize it, see all option and try to understand reason behind each right and wrong option.
