- 1. Service Member Capstone Event Required Documentation Note: There are no Career Readiness Standards for Entrepreneurship Career Path. For the purpose of the Capstone Event the Service member will complete Individual Transition Plan (ITP) Block 5 and present to Transition Counselor for review. Attend Pre-Separation Counseling Complete Pre-Separation Counseling Checklist DD Form 2648 / DD Form 2648-1 (Present DD Form 2648) Register for VA Benefits (eBenefits) (Present eBenefits registration documentation) Complete a Post-Separation 12-month budget reflecting personal and family goals and obligations (Complete TurboTAP Financial Planning Worksheet from the 5-Day Workshop) Worksheet Link: http://www.turbotap.org/export/sites/default/transition/resources/PDF/financial_planning_worksheet_fillable.pdf Evaluate opportunties presented by continuing military service in a Reserve Component ((Present DD Form 2648) Crosswalk military skill set to civilian skills (MOS crosswalk) to include an evaluation of the demand for those civilian skills within the potential relocation destinations (Completed Appendix B from Military Occupation Codes Crosswalk Module from Day 1 of the 5-Day T-GPS Workshop) Identify and document requirements and eligibility for licensure, certification and apprenticeships at the potential relocation destinations (Completed ITP, Section II, item B) Complete the Individual Transition Plan and provide documentation of meeting the Career Readiness Standards for the chosen career path (Completed ITP Block for one of the below career paths or the Entrepreneurship career path) Career Readiness Standards Applicable to all Career Paths *** Complete the DANTES or other DoD-approved standardized career assessment tool selected by your military Service (Complete Kuder Assessment) Prepare and submit the Job Application Package (e.g., create resume, identify references, present at least two employment applications, and/or provide a job acceptance letter) (Present completed Job Application Package or employment acceptance letter) Obtain a Gold Card Certificate from the Department of Labor (Present DoL Gold Card) Employment Career Readiness Standards*** Complete an education needs assessment (Complete Kuder Assessment) Identify, compare, and select academic institutions based on specific selection criteria (Complete ITP Block 3 Section V. Item D.) Prepare and submit an Education Application Package (e.g., submit application to academic institution and/or provide an acceptance letter) (Present completed Education Application Package or institution acceptance letter) Schedule one-on-one counseling with the academic advisor from the institution you will attend (Complete ITP Block 3 Section V. Item D.) Connect with the Student Veteran Organization at your chosen institution (Complete ITP Block 3 Section V. Item D.) Education Career Readiness Standards*** Complete an education needs assessment (Complete Kuder Assessment) Identify, compare, and select technical training institutions based on specific selection criteria (Complete ITP Block 4 Section VI. Item D.) Technical Training Career Readiness Standards*** Prior to completing your Individual Transition Plan (ITP), it is important to note that there are Career Readiness Standards you have to meet. You will be required to provide documentation of meeting the following readiness standards to your Transition Counselor and Command representative prior to separation. These standards are designed to increase your ability to successfully overcome any challenges you may face in pursuit of your chosen career path. Some Career Readiness Standards apply to all career paths (Employment, Education, Technical Training and Entrepreneurship) while others only apply to a specific career path. They are as follows:
