This document contains a 16-item Sources of Feedback Scale questionnaire that measures an employee's perceptions of feedback from four sources: self, task, supervisor, and organization. Respondents are asked to rate statements related to each source on a 7-point scale. The scale aims to assess how much information an employee receives about their performance from these different sources.
