When a patient is diagnosed with cancer at UC Davis’ Comprehensive Cancer Center, an administrative nurse creates a Personal Health Network for the patient and builds out his care team. She can add family members, the primary care doctor, second opinion doctors, and everyone else that needs to participate. Then, she organizes the complete medical record from everywhere that the patient is being treated, ensuring that every member of the care team has access to all the patient’s clinical information regardless of where they practice. Then, when all of the people and information is connected in one place, doctors and nurses use Tiatros’ shared workflow engine to assign paperwork, manage appointments, and track medications. They can send questionnaires directly to the patient’s mobile device to gather accurate, actionable information about the patient’s health, as well as pull data from medical devices. Finally, the patient and his family are always in contact with their entire care network, exchanging secure SMS messages, sharing educational materials in their cloud library, and participating in secure videoconferences.
