According to three reports from January 2004, 45% of firefighter deaths occurred during fireground operations, with the second highest cause being responses or returns at 26%. Overturns were the leading cause of emergency vehicle incidents based on both frequency and severity, accounting for 45% and 29% respectively, with intersections the second highest cause based on severity at 45%.
