This document is a parent's consent and waiver form for a student to participate in events. It contains a waiver signed by the student waiving any liability against authorities for any harm that may occur during participation. It also contains a parent's consent waiving rights to claim against authorities for any incidents that may befall the student during participation or travel to and from events. Forging signatures on the form would be considered a serious offense that could result in expulsion and legal charges.
