This document summarizes the results of a teacher supervision evaluation in three areas: planning administration, learning implementation, and assessment administration. The teacher was evaluated on 10 criteria for planning, 48 criteria for implementation, and 10 criteria for assessment. Scores were provided for each criterion to calculate a total score and percentage achieved. Based on the percentage, the teacher's performance in each area was rated as either excellent, good, sufficient, or lacking. Overall feedback and areas for improvement were also provided.
