Selective serotonin reuptake inhibitors (SSRIs), a common type of antidepressant, were found to be linked to an increased risk of dental implant failure according to a study of 490 patients. The study showed a failure rate of 10.6% for implants in patients taking SSRIs, compared to 4.6% for patients not taking them. A separate cost-effectiveness study of surgical treatments for trigeminal neuralgia found that percutaneous stereotaxic rhizotomy (PSR) was the most cost-effective at $600 per quality adjusted life year, though it was the least commonly performed procedure.

widely used antidepressant drugs—
may be associated with an increased
risk of failure of osseointegrated
implants, according to a study
published online Sept. 3 in Journal of
Dental Research.
Researchers from McGill Univer-
sity in Montreal, Quebec, Canada,
and East Coast Oral Surgery in
Moncton, New Brunswick, Canada,
conducted a retrospective cohort
study of patients treated with dental
implants.
The researchers examined records
from 490 patients who received a
total of 916 implants from January
2007 to January 2013. Eight hun-
dred twenty-two of these implants
were placed in 440 patients who
did not use SSRIs, and 94 implants
were placed in 50 patients who used
SSRIs, the authors wrote.
After three to 67 months’ follow-
up, the researchers found that 38 of
822 implants in the nonusers group
had failed, for a failure rate of 4.6
percent. In the users group, 10 of 94
implants had failed, for a failure rate
of 10.6 percent.
The researchers also reported
that small implant diameters, bone
augmentation and smoking hab-
its seemed to be associated with a
higher risk of implant failure.
One study limitation was that the
investigators could not acquire infor-
mation about drug compliance dose
and treatment period from patients’
files, the authors wrote.

FOR TRIGEMINAL NEURALGIA
UNDERUSED, STUDY FINDS
The most cost-effective surgical
procedure for trigeminal neural-
gia may be the one performed
least often, according to a study
published in the September issue of
Neurosurgery.
Siviero Agazzi, MD, MBA, and
colleagues from the University of
South Florida in Tampa compared
the cost-effectiveness of various
surgical options for trigeminal neu-
ralgia by analyzing Medicare claims
data during 2011.
Of 1,582 patients with trigeminal
neuralgia, 94 (6 percent) under-
went surgical treatment, the authors
wrote. Forty-eight of these patients
underwent microvascular decom-
pression (MVD), 39 underwent
stereotactic radiosurgery (SRS) and
seven underwent percutaneous
stereotaxic rhizotomy (PSR).
The researchers reported that the
average weighted cost of surgery was
about $40,000 for MVD, $38,000 for
SRS and $3,900 for PSR.
Over 10 years, the average quality-
adjusted life-years (QALYs)—reflect-
ing the time free of pain without
medications—were 8.2 for patients
undergoing MVD, 4.9 for those
undergoing SRS and 6.5 for those
undergoing PSR, the authors wrote.
After accounting for these dif-
ferences, the researchers found that
PSR was the most cost-effective
option. The reported cost per QALY
gained was about $600 for PSR,
$4,900 for MVD and $7,800 for SRS.
Dr. Agazzi and coauthors empha-
sized the preliminary nature of their
study. However, the results suggest
that PSR is an underutilized pro-
cedure for patients with trigeminal
neuralgia who require surgery, wrote
the authors. ■

This study investigated the surgical benefits of using protoporphyrin IX (PpIX) fluorescence to detect meningioma tissue in 204 patients. The study found that PpIX fluorescence enabled visualization of most meningiomas and identification of residual tumor tissue. However, the study had limitations including its single center design and lack of histopathological correlation or analysis of recurrence rates. While the results suggest PpIX fluorescence may help reduce recurrence risks, more research is needed to determine its true benefits versus potential harms for changing clinical practice.

Radiation-induced meningiomas (RIM) are more likely to be multiple and recur compared to spontaneous meningiomas. The studies reviewed found that 18% of RIM cases were multiple and 23% recurred, compared to lower rates for spontaneous meningiomas. While the studies could not definitively conclude RIM are more likely to be higher grade, the majority were found to be WHO grade 1 or 2. Long-term surveillance is recommended for individuals who received cranial radiation, such as childhood cancer survivors, due to the increased risk of developing RIM. Further research is needed to better understand RIM characteristics and natural history.

Titanium cranioplasty may be superior to autologous bone cranioplasty based on a randomized controlled trial comparing the two procedures. The trial found that titanium cranioplasty resulted in fewer implant failures requiring reoperation compared to autologous bone cranioplasty, though the difference was not statistically significant. Titanium cranioplasty also led to better reported functional and cosmetic outcomes with no difference in healthcare costs between the two groups over one year of follow up. However, the trial was prone to bias because the surgeon was not blinded to the treatment group assignment. Larger, multi-center studies are needed to verify the results.

This multicenter study examined 503 patients with recurrent glioblastomas who underwent surgical resection. It found that aggressive surgical resection of recurrent glioblastomas, when followed by adjuvant therapy, was associated with increased overall survival. Specifically, survival was longer for patients who were younger, had a higher Karnofsky performance status, underwent a more extensive resection, and had a longer time between initial diagnosis and recurrence. However, the study had limitations as a retrospective analysis subject to selection bias. Repeat resection improved survival, but its benefits compared to nonsurgical therapy require further prospective evaluation.

However, the study had limitations as a retrospective single-center analysis subject to selection bias. The study evaluated long-term outcomes of staged-volume stereotactic radiosurgery for large arteriovenous malformations. 61% of patients achieved total obliteration of the malformation. Seizure control and neurological deficits were not significantly improved by treatment. However, the study had a prospective case series design without controls and was subject to selection, information, and attrition biases. Statistical analyses did not include tests to compare outcome curves. The results do not clearly demonstrate benefits of the fractionated radiosurgery approach for large or symptomatic AVMs.

This study compared the union rates of tibiotalocalcaneal (TTC) arthrodesis using an intramedullary nail with either internal or external bone stimulation. The study found that the union rates were similar between internal (53%) and external (57%) bone stimulation. Additionally, the rates of successful fusion capable of independent ambulation were similar between internal (81%) and external (83%) groups. While time to partial weight bearing was slightly faster with internal stimulation, there were no other significant differences in outcomes between the two bone stimulation methods.

Patients were treated with surgical curettage, calcitonin, interferon, or a combination. Surgical curettage alone had a 29% recurrence rate. Calcitonin and combinations of calcitonin and interferon did not result in any recurrences. Some patients required additional limited surgery after pharmacological treatment to correct facial contours or remove remaining lesions, but none of these recurred. The study concludes that pharmacological treatments are effective for stabilizing or reducing CGCG tumors, with lower recurrence rates than surgery alone. Further research is still needed to determine the best pharmacological agent for individual tumors

This study evaluated the use of the Anatomage Table, a 3D virtual reality tool, to teach cerebrovascular anatomy to neurology residents. Five residents completed pre- and post-tests on cerebrovascular structure identification before and after traditional textbook-based teaching and hands-on use of the Anatomage Table. Test scores improved significantly from a average of 32% to 88% after using the virtual reality tool. A post-survey found that residents strongly agreed that the Anatomage Table enhanced their ability to identify cerebrovascular structures in 3D. The results suggest that interactive 3D virtual reality tools can effectively complement traditional teaching methods for neuroanatomy.

A 12-year-old female presented with swelling and pain in the right lower back tooth region. Radiographs revealed a well-defined radiolucency surrounding the right mandibular first, second, and third molars. The lesion was enucleated along with extraction of the involved teeth. Histopathological examination found a cystic cavity lined by non-keratinized epithelium, consistent with a radicular cyst. Follow-up after 1 month showed complete healing. This case report describes the presentation, management, and outcomes of a large multi-tooth radicular cyst in the mandible of a pediatric patient.

The study reviewed records of 75 patients who received 155 implants. Implant survival rates after 12 months were significantly lower for smokers (79%) compared to non-smokers (87%). Analysis also showed smoking had a more significant negative effect on implant survival when the preoperative bone height was less than 4 mm, with survival rates of 82.4% for non-smokers and 60% for smokers. The study concludes smoking should be considered a high risk factor for implants placed immediately in grafted sinuses, especially in areas of limited bone height.

Introduction: The main objective of endodontic therapy is the complete elimination of microbes and their by-products from the root canal system. Despite the high success rate of root canal treatment, there are failures too. Hence, our survey aimed to investigate the differences in cleaning, shaping, and obturation protocols followed among general dentists and endodontists in India. Method: A multiple choice questionnaire was designed and distributed to 700 dentists in India through a social media application. The questions covered all the protocols and techniques for root canal treatment. The collected data was analyzed using descriptive statistics at a 0.05 level of significance to get the results as frequencies and percentages. Results: A total of 332 dentists among 700 participated in the survey. Among 332 dentists, 181 were non-endodontists and 149 were endodontists. The highest number of participants used K- hand files for instrumentation followed by NiTi rotary instruments in a torque with gear reduction handpiece following the Step-back technique. Participants polled for calcium hydroxide as a choice of interappointment intracanal medicament in case of lesion and the patients were recalled after 7 days. 99.69% of participants favored the usage of gutta-percha with zinc oxide eugenol for root canal obturation. Most of the participants used composite restoration as postendodontic restoration and advocated crowns in all cases Conclusions: Though there were only 332 participants in this survey, the study was able to provide the details of the dental practice among the population. This survey could also show the standard of care, knowledge, and skills that have improved due to continual dental education through conferences, seminars, and workshops. However, further studies are necessary to evaluate this trend.

An assignment for DH Theory 1 was to create an annotated bibliography for a research topic of our choice. This project was designed to challenge our critical thinking and decision making skills. I chose to research dental lasers and their effectiveness in the removal of oral lesions. It was interesting to compare different literature on this subject, and I had a lot of fun determining what articles were valid based on the evidence provided.

The papillae scores improved on average from 0.8 to 2.4 after 16 months, demonstrating regeneration of the papillae over 11-30 months. This study assessed the 5-year outcome of apical microsurgery in 170 teeth from 191 subjects who underwent the procedure. At the 5-year follow-up, 129 teeth (75.9%) were healed compared to 83.8% at 1 year. Two significant predictors of healing outcome were identified: teeth with interproximal bone loss >3 mm from the cementoenamel junction had a lower healing rate (52.9%) compared to those with ≤3 mm bone loss (78.2%); and teeth filled with ProRoot MTA had a higher healing rate (86.4%) than those filled with SuperEBA (67.3%).

On average, bone loss was higher in the maxilla than mandible but remained below 1.5 mm in both arches. A previous smoking habit and implants with low insertion torque that were delayed in loading showed greater bone loss compared to immediately loaded implants with high insertion torque. While panoramic radiographs have limitations, mean bone loss was comparable to standard-diameter implants and remained clinically insignificant.

Immediate implants placed at the time of extraction required bone augmentation more often than implants placed 12 weeks later (72% vs 43.9%). Delayed implants were less likely to have wound failures and exhibited better soft tissue outcomes. Based on the need for more frequent bone grafting and poorer aesthetic results, the study concludes delayed implant placement may be preferable to immediate placement in the aesthetic zone.

This study evaluated the success of a crestal approach sinus lift procedure combined with immediate implant placement in patients with varying amounts of residual alveolar bone (RAB). 102 patients received 109 implants across three clinical sites. Patients were divided into two groups based on RAB height: group 1 had 24 mm RAB and group 2 had >4 mm RAB. The success rate was 100% for group 1 and 98.5% for group 2 after 6-100 months. Both groups experienced minimal crestal bone loss on average (0.55 mm for group 1 and 0.07 mm for group 2) with no significant difference between groups. This study evaluated 42 dental implants placed with high insertion torques of 70 Ncm or greater. All implants successfully integrated clinically and were stable after 1 year of loading. Marginal bone loss after 1 year was similar between implants placed with high torques (mean 1.24 mm) and low torques (mean 1.09 mm), indicating that high insertion torques did not negatively impact osseointegration or bone stability. The use of high insertion torques up to 176 Ncm did not prevent osseointegration and resulted in similar bone stability outcomes compared to lower torque implants.

This document summarizes a review of literature on patient-related risk factors for dental implant therapy. The review identified 43 relevant studies based on inclusion criteria. The studies contained confounding variables and small sample sizes in subgroups. Consistent evidence showed increased implant failure rates in smokers, patients with a history of radiotherapy, and those with poor local bone quality or quantity. Weaker evidence linked a history of periodontitis to higher peri-implant disease. A lack of evidence precluded guidelines for patients with autoimmune disorders. Emerging evidence suggests a correlation between genetic traits and disrupted osseointegration.

Objectives: 1.To clinically evaluate the healing process following periapical surgery with Chitra granules 2.To radiographically evaluate healing, following periapical surgery with Chitra granules. 3.To compare the bone healing in the study group, clinically and radiographically following the modalities of treatment, with that of the control group (without graft). Materials: 1.Chitra hydroxyapatite granules. 2. Gutta-percha — for root canal obturation by lateral condensation. 3. High Copper amalgam used as retrofilling materials. To evaluate healing after periapical surgery using Chitra granules, 22 patients were selected from out patient section of Department of Conservative Dentistry and Endodontics, Govt. Dental College, Kozhikode,based on clinical and radiographical evaluation. The study group consisted of 13 males and 9 females. All were of the age group 15-35 years. After selection they were randomly divided into two groups A and B. In group A the Chitra granule were placed in the bony defect and in group B the defect were left as such after surgery. Results: Clinical parameters showed better early symptom free condition in group A compared to group B. But data found statistically insignificant (t-7.27,df-1.8, P >0.05). Radiographical evaluation data analysis showed statistically significant difference among group A and group B. Conclusion: Biocompatible Chitra granule not only obliterates the cavity but act as a scaffold for bone growth and prevent scar tissue formation.It is osteoconductive. In comparison to the conventional periapical surgery, the placement of Chitra granules facilitates bone regeneration more easily. The material is found to be very cost effective, easily available, easy to manipulate and involves least complication to both clinicians and patients.

1. 1016 JADA 145(10) http://jada.ada.org October 2014 NEWS ANTIDEPRESSANTS MAY BE LINKED TO IMPLANT FAILURE RISK, RESEARCHERS REPORT Selective serotonin reuptake inhibitors (SSRIs)—the most widely used antidepressant drugs— may be associated with an increased risk of failure of osseointegrated implants, according to a study published online Sept. 3 in Journal of Dental Research. Researchers from McGill Univer- sity in Montreal, Quebec, Canada, and East Coast Oral Surgery in Moncton, New Brunswick, Canada, conducted a retrospective cohort study of patients treated with dental implants. The researchers examined records from 490 patients who received a total of 916 implants from January 2007 to January 2013. Eight hun- dred twenty-two of these implants were placed in 440 patients who did not use SSRIs, and 94 implants were placed in 50 patients who used SSRIs, the authors wrote. After three to 67 months’ follow- up, the researchers found that 38 of 822 implants in the nonusers group had failed, for a failure rate of 4.6 percent. In the users group, 10 of 94 implants had failed, for a failure rate of 10.6 percent. The researchers also reported that small implant diameters, bone augmentation and smoking hab- its seemed to be associated with a higher risk of implant failure. One study limitation was that the investigators could not acquire information about drug compliance dose and treatment period from patients’ files, the authors wrote. This study was supported by the China Scholarship Council, Clifford Wong Fellowship, Canadian Insti- tutes of Health Research, Institute of Musculoskeletal Health and Arthritis Bridge Funding, and the Network for Oral and Bone Health Research. COST-EFFECTIVE PROCEDURE FOR TRIGEMINAL NEURALGIA UNDERUSED, STUDY FINDS The most cost-effective surgical procedure for trigeminal neuralgia may be the one performed least often, according to a study published in the September issue of Neurosurgery. Siviero Agazzi, MD, MBA, and colleagues from the University of South Florida in Tampa compared the cost-effectiveness of various surgical options for trigeminal neuralgia by analyzing Medicare claims data during 2011. Of 1,582 patients with trigeminal neuralgia, 94 (6 percent) underwent surgical treatment, the authors wrote. Forty-eight of these patients underwent microvascular decompression (MVD), 39 underwent stereotactic radiosurgery (SRS) and seven underwent percutaneous stereotaxic rhizotomy (PSR). The researchers reported that the average weighted cost of surgery was about $40,000 for MVD, $38,000 for SRS and $3,900 for PSR. Over 10 years, the average quality- adjusted life-years (QALYs)—reflect- ing the time free of pain without medications—were 8.2 for patients undergoing MVD, 4.9 for those undergoing SRS and 6.5 for those undergoing PSR, the authors wrote. After accounting for these differences, the researchers found that PSR was the most cost-effective option. The reported cost per QALY gained was about $600 for PSR, $4,900 for MVD and $7,800 for SRS. Dr. Agazzi and coauthors empha- sized the preliminary nature of their study.
