Critical problem of modern medical informatics are: fragmented information, documents and patient data stored in a large number of information systems that are not connected. Another problem is lack of unified international standards in the field of medical informatics, communication and exchange of data. This situation presents a major problem for medical staff on one side and the patient on another. Patient has big problems to get access and collect his medical records. On the other hand medical workers always work on partial patient data, the one that they have in their own system. Crucial data for patient treatment that can critically affect the healing process are scattered in the hospital, medical and specialist information systems.
Company Parsek d.o.o. designed an integrated healthcare portal - Vitaly, allowing the patient and medical staff have access to all data in the form of intuitive web portal. Vitaly is safe, user-friendly, web-based tool that connects and unites stakeholders, information and medical documentation. Vitaly connect stakeholders in process prior to treatment, during the treatment process and in the post treatment process. Vitaly allows and encourages interaction between patients and medical staff; the exchange of documents, messages, results of examinations, effects of medications and the patient's well-being. Communication between Vitaly and other information systems is based on international standards in the field of medical informatics (IHE and HL7) and a special focus on secure and encrypted exchange of all information and documents.
Nujnost odličnih specifikacij in kako jih zapisati (Miloš Cigoj, NTK 2016) ParsekPredpogoj za dolgoročni uspeh katerega koli projekta je čim boljše zajetje uporabniških potreb in zapis le teh v funkcionalno specifikacijo. Brez te bomo težko merili uspešnost izvedbe, se pogajali z naročnikom in vzdrževali rešitev. Žal še vedno obstaja veliko ekip, ki ta korak preprosto izpusti in direktno skočijo na kodiranje. Izkušnje kažejo, da ni učinkovitejšega načrtovanja in modeliranja kot z uporabo naravnega opisa domene. Naravni jezik nam omogoča tudi hitrejše odpravljanje nelogičnosti že predenj pridemo do razvoja, kar je veliko cenejše. Med nami so tudi ekipe, ki specifikacijo zapišejo in je sama sebi namen, kajti je zapisana v obliki, da jo je nemogoče obvladovati in iz nje razbrati kaj sploh rešujemo naročniku, kaj šele “navodila” za izvedbo in verifikacijo. Take specifikacije so žal celo potrjene pri naročniku a obstanejo nekje na polici. Res je, da vsak projekt zahteva drugačen pristop in zato so tudi funkcionalne specifikacije v drugačnih oblikah, ampak naj bodo žive in naj odražajo dejansko potrebo in implementacijo. Cilj predavanja je predstaviti zakaj so funkcionalne specifikacije pomembne, kakšni so slabi in dobri primeri in navodila za pisanje specifikaciji. Predstavil bom več načinov pisanja specifikaciji, njihove prednosti in slabosti.

Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

Povzetek
Rak rana sodobne medicinske informatike so razdrobljene informacije, dokumenti in podatki o
bolniku, ki so shranjeni v velikem številu informacijskih sistemov, ki med seboj niso povezani.
Dodatni problem predstavlja dejstvo, da informacijski zdravstveni sistemi ne podpirajo mednarodnih
standardov s področja medicinske informatike, zato komunikacija in izmenjava podatkov med njimi ni
možna. Takšno stanje predstavlja velik problem tako za medicinsko osebje kot tudi bolnika, ki težko
dobi dostop do svoje medicinske dokumentacije. Medicinski podatki, ki so ključni za obravnavo
bolnika in lahko kritično vplivajo na proces zdravljenja so razpršeni v bolnišničnih, zdravstvenih in
specialističnih informacijskih sistemih.
Podjetje Parsek d.o.o. je zasnovalo enotni zdravstveni portal – Vitaly, ki omogoča bolniku in
medicinskemu osebju dostop do vseh podatkov v obliki intuitivnega spletnega portala. Vitaly je varno,
uporabniku prijazno, spletno orodje, ki povezuje in združuje deležnike, informacije in medicinsko
dokumentacijo pred pričetkom procesom zdravljenja, med samim procesom in po zaključenem
procesu zdravljenja. Vitaly omogoča in spodbuja interakcijo med bolnikom in zdravstvenim osebjem;
z izmenjavo dokumentov, sporočil, rezultatov preiskav, učinkovanja zdravil in bolnikovega počutja.
Komunikacija med produktom Vitaly in ostalimi informacijskimi sistemi je zasnovana na
mednarodnih standardih s področja medicinske informatike (IHE, HL7, FHIR) in posebnim
poudarkom na varni in šifrirani izmenjavi podatkov in dokumentov.
Abstract
VITALY – HEALTHCARE WEB PORTAL
Critical problem of modern medical informatics are: fragmented information, documents and patient
data stored in a large number of information systems that are not connected. Another problem is lack
of unified international standards in the field of medical informatics, communication and exchange of
data. This situation presents a major problem for medical staff on one side and the patient on another.
Patient has big problems to get access and collect his medical records. On the other hand medical
workers always work on partial patient data, the one that they have in their own system. Crucial data
for patient treatment that can critically affect the healing process are scattered in the hospital,
medical and specialist information systems.
Company Parsek d.o.o. designed an integrated healthcare portal - Vitaly, allowing the patient and
medical staff have access to all data in the form of intuitive web portal. Vitaly is safe, user-friendly,
web-based tool that connects and unites stakeholders, information and medical documentation. Vitaly
connect stakeholders in process prior to treatment, during the treatment process and in the post
treatment process. Vitaly allows and encourages interaction between patients and medical staff; the
exchange of documents, messages, results of examinations, effects of medications and the patient's
well-being. Communication between Vitaly and other information systems is based on international
standards in the field of medical informatics (IHE and HL7) and a special focus on secure and
encrypted exchange of all information and documents.

1. UVOD
Napredne tehnologije omogočajo konstantno spremljanje in shranjevanje različnih informacij,
ki se lahko v realnem času tudi analizirajo. Pri čemer je področje medicine eno izmed najmanj
informatiziranih. V medicini še vedno niso uporabljene tehnološke zmožnosti, ki so že
preizkušene in bi lahko pripomogle h preprečevanju bolezni, hitrejšemu odkrivanju bolezni,
učinkovitem zdravljenju in boljšemu okrevanju bolnikov. Najbolj pogosto se napredne
tehnološke naprave uporabljajo lokalno v posameznih zdravstvenih institucijah. Pri čemer se
uporabijo šele, ko so določeni kritični parametri presegli mejne vrednosti, zaradi česa je,
posameznik že postal bolnik.
Na podlagi »post festum« meritev, ki jih danes opravlja medicinsko osebje, lahko le
deduktivno sklepa kaj se je zgodilo in s pomočjo rekonstrukcije definira način zdravljenja
pacienta. Retrogradna diagnostika ima številne pomanjkljivosti in zahteva veliko visoko
kvalificiranih zdravstvenih delavcev. Najbolj pogosto uporabljene diagnostične naprave so v
veliki večini primerov sposobne evidentirati le enega izmed številnih parametrov, ki vplivajo
na zdravje osebe, oz. tem primeru že bolnika. Posledično so informacije shranjene v različnih
specialističnih informacijskih sistemih in bolniki so odgovorni za prenos dokumentov in
informacij. Razdrobljenost medicinskih informacijskih sistemov povzroča: izgube podatkov
in dokumentov ter anomalije v anamnezi pacienta in procesu zdravljenja. V raziskovalni hiši
Mckinsey izpostavljajo dostopnost in razpoložljivost medicinskih podatkov kot osrednji
katalizator prenove zdravstvenega sistema. »Preglednost in dostopnost medicinskih podatkov
ima potencial za povečanje odgovornosti, produktivnosti in kakovosti storitev, povečanje
aktivnega sodelovanja bolnikova v zdravstveni oskrbi, in spodbujanje gospodarske rasti«.
Naslednji seznam zajema le nekaj večjih informacijskih sistemov, kjer so shranjeni podatki o
bolniku:
a) Splošni zdravnik (privatna organizacija / javna ustanova)
b) Bolnišnice (privatna organizacija / javna ustanova)
c) Laboratorij (privatna organizacija / javna ustanova)
d) Specialistične ambulante (privatna organizacija / javna ustanova)

Centralno vozlišče oz. hrbtenica, ki le poveže različne medicinske informacijske sisteme je
predpogoj, vendar brez podpore standardom in brez predstavitve dostopa do podatkov na
uporabniku prijazen način ne prinaša dodane vrednosti. Podatki, ki so na voljo v različnih
zdravstvenih sistemih povezani preko centralnega vozlišča in podatki bolnika iz drugih virov
morajo biti predstavljeni vsem deležnikom na enostaven, intuitiven in moderen način. Sistem
mora omogočiti bolniku upravljanje medicinskih podatkov in komunikacijo z zdravstvenimi
delavnici preko enotnega vmesnika. Na drugi strani mora sistem omogočiti zdravstvenim
delavcem celovito obravnavo bolnika in pregled nad vsemi informacijami.
2. SPLETNI ZDRAVSTVENI PORTAL - VITALY
Podjetje Parsek d.o.o. je zasnovalo enotni zdravstveni portal – Vitaly, ki omogoča bolniku in
medicinskemu osebju dostop do vseh podatkov v obliki intuitivnega spletnega portala 5. Na
sliki 1 je predstavljen koncept portala Vitaly in na sliki 2 je predstavljen informacijski
ekosistem, ključni deležniki in protokol komunikacije spletnega zdravstvenega portala. Vitaly
postavlja v središče osebo – uporabnika portala, ki je lahko pacient oz. zdravstveno osebje in
mu omogoča enostaven način komunikacije, dostopa in celovite obravnave. Portal je zgrajen
modularno in se ga lahko prilagodi glede na potrebe naročnika. Uporablja se lahko kot
samostojna rešitev oz. se integrira kot storitev. Modularna zasnova Vitaly.a temelji na
portalski tehnologiji in omogoča izdelavo zaključenih funkcionalnosti v obliki neodvisnih
modulov (eng. portlet). Administrator portala lahko posamezne module aktivira glede na
potrebe naročnika z vidika ustanove oz. posameznega uporabnika. Vitaly vključuje večjezično
podporo. To pomeni, da je možno vsak modul kot tudi celotno rešitev hitro in enostavno
prevesti v drug jezik. Modularna zasnova spletnega portala in možnost prilagoditve portala za
posameznega uporabnika racionalizira klinične in poslovne delovne tokove in motivira
bolnike, da se aktivno vključijo v proces zdravljenja. Vitaly omogoča hitrejšo in natančnejšo
komunikacijo med zdravstvenim osebjem in bolnikom. S pomočjo spletnega portala lahko
bolnik komunicira neposredno z osebnim zdravnikom oz. medicinsko sestro. Na drugi strani
lahko zdravnik pregleduje meritve vitalnih znakov bolnika, ki je v domači oskrbi in mu poda
navodila za prilagoditev rehabilitacije glede na meritve prenosnih diagnostičnih naprav, ki so
povezane v portal. Vitaly spodbuja vzpostavitev aktivnega vzajemnega sodelovanja med
bolnikom in zdravstvenimi delavci. Dodatno portal omogoča bolniku izmenjavo informacij po
njegovi presoji z drugimi zdravniki specialisti in njegovo družino.

3. MODULI PORTALA VITALY
Unikatna modularna zasnova omogoča veliko prilagodljivost posameznih funkcionalnosti in
celotne portalske rešitve. Vsako izmed portalskih funkcionalnosti, ki so opisane v
nadaljevanju, se lahko prilagodi za različne uporabniške profile (bolnik, zdravnik, medicinska
sestra in drugo zdravstveno osebje), bolezen (primer: diabetiki, srčno popuščanje, ipd) oz.
segment bolnikov (primer: otroci, starejši, ipd). Vsak portalski modul ima pripravljeno
predlogo primerov uporabe s seznamom podprtih mednarodnih standardov iz varnosti in
medicine informatike. Vsaka komunikacija na portalu se beleži s pomočjo spletnih in
strežniških dnevniških zapisov. Bolnik lahko v vsakem trenutku v spletnem portalu preveri
kdo je pregledoval in urejal njegovo medicinsko dokumentacijo in druge medicinske oz.
osebne podatke.
Spletni portal Vitaly sestavljajo celoviti moduli, ki so razdeljeni v dve skupini: osnovne in
napredne funkcionalnosti. Administrator portala lahko prilagodi in vključi/izključi posamezno
funkcionalnost glede na potrebe in pravice uporabnika portala. Pravice uporabnikov se urejajo
na preprost način s centralnim sistemom vlog. Pri čemer portal podpira sistem enotne prijave
in integracije z drugimi informacijskimi sistemi oz. ponudniki podatkov.
Osnovne funkcionalnosti vključujejo:
a) Vstopna stran (eng. dashboard). Primer vstopne strani bolnika je predstavljen na sliki .

b) Osebni profil (osebni in medicinski podatki, medicinske opombe).
c) Osebni dnevnik.
d) Seznam mojih zdravnikov in drugih zdravstvenih delavcev.

g) Interaktivni zemljevid z naprednim iskalnikom zdravstvenih ustanov.
Napredne funkcionalnosti vključujejo:
a) Komunikacijski modul (eng. inbox).
b) Modul za pregledovanje in urejanje medicinske dokumentacije.
c) Modul za spremljanje vitalnih znakov s pomočjo prenosnih diagnostičnih naprav.

4. KLJUČNE PREDNOSTI PORTALA VITALY
S pomočjo portala Vitaly lahko bolnik hitreje in učinkoviteje komunicira z zdravstvenim
osebjem. Tako v okviru procesa zdravljenja, rehabilitacije kot tudi preventivnih
aktivnosti,Vitaly omogoča, da ima bolnik na enem mestu shranjene vse podatke in
dokumente, ki zadevajo njegovo osebno zdravje oz zdravje njegove družine. Portal se lahko
uporablja za beleženje bolnikovih fizičnih in družbenih dejavnosti, prehranskih navad, zdravil
in drugih aktivnosti, ki na kakršen koli način vplivajo na njegovo zdravje. Uporabnik ima
popolno kontrolo nad dostopom in izmenjavo medicinskih podatkov in dokumentov. Sam se
lahko odloči, da določenih dokumentov oz. meritev ne bo delil s svojo družino ali
medicinskim osebjem.
Modul za spremljanje vitalnih znakov s pomočjo prenosnih diagnostičnih naprav omogoča
bolnikom, da samostojno spremljajo in beležijo različne fizikalne parametre, ki se na podlagi
medicinsko certificiranih naprav in varnostnega protokola prenašajo v spletni portal. Meritve
lahko bolnik opravi samostojno oz. s pomočjo medicinskega osebja, ki je dostopno preko
komunikacijskega vmesnika in pripravljeno za video oz. pisno komunikacijo. V portalu lahko
medicinsko osebje nastavi osebne kriterije, limite in opozorila za vsakega bolnika posebej. Na
takšen način lahko sistem pomaga medicinskemu osebju natančneje in aktivno spremljati
proces zdravljenja. Dodatno ima sistem vgrajen protokol varovalk za primere zlorab in

posredovanja napačnih informacij. Kritične informacije so izpostavljene v okviru kratkih
integriranih povzetkov, ki so pripravljeni na podlagi medicinskih protokolov in v vsakem
trenutku omogočajo podrobnejši vpogled v zgodovino merjenja vitalnih znakov, medicinske
dokumentacije in osebne oz. družinske anamneze bolnika. Portal podpira pripravo poročil, ki
vključujejo primerjavo med različnimi vitalnimi znaki in drugimi viri podatkov in temeljijo na
kliničnih protokolih, ki jih definira medicinsko osebje. Ti protokoli so zapisani v obliki
delovnih tokov in se lahko prilagodijo glede na potrebe profila medicinskega uporabnika oz.
klinične obravnave.
ZAKLJUČEK
Ključni problemi dosedanjih medicinskih portalskih rešitev so bili zgrešeni koncepti in ne-
intuitivne rešitve, ki niso omogočale osebne obravnave in osebnih nastavitev. Sodobni
zdravstveni portal mora biti veliko več kot le centralni pregledovalnik medicinske
dokumentacije. Tako bolniki kot tudi medicinsko osebje potrebujejo enostavno orodje, ki je
prilagojeno novim načinom obravnave bolnikov in sodobnemu načinu življenja. Na drugi
strani je zdravstveno osebje preobremenjeno z administrativnimi obveznostmi, ki jih lahko
prevzame direkten prenos podatkov iz prenosnih diagnostičnih naprav in direktna
komunikacija z bolnikom. Takšni primeri so: redno periodično preverjanje vitalnih prametrov
in avtomatsko obveščanje zdravstvenega osebja, ko so bile presežene osebne mejne vrednosti.
S pomočjo prenosnih diagnostičnih naprav se lahko že danes avtomatizirajo deli poslovnih
procesov medicinskega osebja, ki lahko prihranek časa nameni kvalitetnejši oskrbi in negi
bolnikov.
Intuitivni spletni portal Vitaly omogoča vzpostavitev aktivnega vzajemnega sodelovanja med
bolnikom in zdravstvenim osebjem. Izboljšuje komunikacijo s številnimi izvajalci
zdravstvenih storitev in omogoča hitrejše in učinkovitejše izvajanje zdravstvenih storitev.
Hkrati prinaša možnost vzpostavitve novih zdravstvenih protokolov, znižanje birokratskih in
drugih ovir ter reorganizacijo zdravstvenih storitev.
